A 6-year-old child is brought to clinic for persistent enuresis occurring both during the day and at night. The caregiver reports long-standing chronic constipation with infrequent, hard stools and occasional fecal soiling. On physical examination, there is a sacral dimple with a small tuft of hair over the lumbosacral region; lower-extremity strength appears symmetric but deep tendon reflexes are slightly decreased at the ankles. The caregiver asks what should be done next.
