A 1-year-old boy is brought to clinic for 2 weeks of progressive lower-limb weakness and reduced spontaneous leg movements. He has poor feeding and intermittent irritability. Examination shows decreased tone and strength in both legs with diminished patellar reflexes. A firm, non-tender abdominal mass is palpated in the left upper quadrant. CT abdomen/pelvis shows a heterogeneous retroperitoneal paraspinal mass near the left adrenal with punctate calcifications and extension through a neural foramen into the spinal canal, causing cord compression.
