A 38-year-old woman has several months of intermittent blurred vision and progressive unsteadiness while walking. Two years ago, she had an episode of painful monocular visual loss that resolved over a few weeks. Neurologic examination shows mild dysmetria on finger-to-nose testing and a broad-based gait. Brain MRI shows multiple ovoid T2 hyperintense lesions in the periventricular and deep white matter. Spinal MRI shows scattered T2 hyperintense lesions within the cord.
