A 68-year-old man with HIV infection has been poorly adherent to antiretroviral therapy. Over 2 years, he has developed progressive memory complaints, reduced ability to manage daily activities, slowed thinking and movements, and poor attention and concentration. Neurologic examination shows psychomotor slowing with impaired attention and learning, while language and visuospatial skills are relatively preserved. There is no history of stepwise decline, focal neurologic deficits, prominent visual hallucinations, or marked fluctuations in alertness. What is the most likely diagnosis?
