A 45-year-old man with no significant past medical history is evaluated in the hepatology clinic after being recently diagnosed with chronic hepatitis C virus (HCV) infection. He is asymptomatic and denies alcohol use or other drug use. He is treatment-naive. Laboratory testing shows HCV RNA of 2,000,000 IU/mL, genotype 4. CBC and creatinine are within normal limits. Liver enzymes are mildly elevated (ALT 89 IU/L, AST 56 IU/L). Abdominal ultrasound shows a normal liver without cirrhosis or focal lesions. Transient elastography (FibroScan) demonstrates advanced fibrosis with a fibrosis score of F3, but no evidence of decompensation.
