A 66-year-old man presents 12 hours after sudden onset of left-sided arm and leg weakness and slurred speech without improvement. He has hypertension and hyperlipidaemia and is not taking anticoagulants. Examination shows right gaze preference, left facial droop, and 2/5 power in the left upper and lower limbs. Non-contrast CT head shows no intracranial haemorrhage. CT angiography/perfusion shows a proximal right middle cerebral artery occlusion with a small-to-moderate infarct core and a large salvageable penumbra. What is the most appropriate management?
