A 28-year-old G2P0 woman at 10 weeks’ gestation presents with 5 days of severe nausea and repeated vomiting and 1 week of vaginal spotting. The uterus is enlarged to about 16 weeks’ size. Speculum examination shows an irregular, friable tissue mass protruding through the cervical os that bleeds on contact. Serum β-hCG is markedly elevated for gestational age. Transvaginal ultrasound shows a heterogeneous intrauterine mass with multiple cystic spaces and no identifiable fetus. What is the most appropriate next step in management?
