A 10-year-old boy has 2 days of gradually increasing right scrotal pain and swelling, worse with activity. Pain is maximal at the upper pole. Examination shows an erythematous, mildly oedematous right hemiscrotum; the testis lies vertically. There is maximal superior pole tenderness with a small, very tender palpable nodule. The cremasteric reflex is present bilaterally. What is the most likely diagnosis?
