He now has a diffuse, nonpruritic erythematous maculopapular rash on the trunk and extremities without urticaria or mucosal involvement. Examination shows enlarged exudative tonsils, posterior cervical lymphadenopathy, and mild splenomegaly. A rapid streptococcal antigen test done before antibiotics was negative.
