A 23-year-old nulliparous woman has 1 year of progressively worsening crampy lower abdominal pain that starts 1–2 days before menses and continues through the first 3 days of bleeding, now affecting work and sleep. Cycles are regular with moderate flow and no intermenstrual bleeding. She is sexually active, uses condoms inconsistently, and would accept hormonal contraception. Pelvic examination shows a normal-sized, anteverted, non-tender uterus with no adnexal masses or cervical motion tenderness. Urine pregnancy test is negative.
