A 40-year-old woman has 12 years of symptomatic lower-limb varicose veins with bilateral leg heaviness and ankle swelling that worsen with prolonged standing and improve with leg elevation. Examination shows varicosities along the great saphenous vein distribution with mild pitting ankle edema and no skin pigmentation or ulceration. Duplex ultrasound shows primary great saphenous vein reflux with a competent deep venous system and no thrombosis. She has not used compression therapy and asks for definitive symptom relief.
