A 62-year-old woman with type 2 diabetes mellitus, hypertension, and dyslipidaemia presents to the emergency department with 3 hours of central continuous chest pain. She had an ischaemic stroke 1 month ago. She denies dyspnoea or palpitations. Home medications include insulin, lisinopril, metoprolol, atorvastatin, and aspirin. ECG shows 2–3 mm ST-segment elevation in leads II, III, and aVF with reciprocal ST depression in I and aVL. She has received oxygen, morphine, and sublingual nitroglycerin.
