A 32-year-old nulliparous woman presents with heavy, regular menstrual bleeding for 4 months, changing pads every 1–2 hours on the heaviest days. She has no intermenstrual bleeding, pelvic pain, dyspareunia, or postcoital bleeding. She wants to preserve fertility. Pelvic examination shows a normal-sized, non-tender uterus with no adnexal masses. Transvaginal ultrasound shows a single 4 × 4 cm subserosal uterine leiomyoma with no intracavitary distortion; endometrial thickness and ovaries are normal. What is the most appropriate management?
