A 7-year-old child has had 6 episodes of sore throat over the past 12 months, each with fever and painful swallowing, treated as tonsillitis with antibiotics. For 6 months, the caregiver reports loud nightly snoring, mouth breathing during sleep, and occasional witnessed pauses in breathing. Oropharyngeal examination shows bilaterally enlarged tonsils (grade 3) without uvular deviation or peritonsillar swelling. There are no craniofacial anomalies or neuromuscular disease. What is the most appropriate management?
