A 28-year-old G2P1 at 33 weeks’ gestation is in the second stage of labor. Pregnancy has been uncomplicated and the fetal heart rate tracing is reassuring. The cervix is fully dilated, membranes are ruptured, and the fetal head is visible at the introitus in occiput anterior position. There is no suspected cephalopelvic disproportion. After prolonged pushing, she is exhausted and cannot continue effective expulsive efforts. What is the most appropriate next step in management?
