A 72-year-old woman presents with a progressively enlarging bulge at a prior midline laparotomy scar. She underwent an open sigmoidectomy 3 years ago. The bulge was previously small and reducible, but over the past 2 months it has increased in size and causes pain during activity. She has no nausea, vomiting, or change in bowel habits. Examination shows a 5–6 cm reducible midline incisional hernia with a cough impulse and intact overlying skin. What is the most appropriate management?
