A 35-year-old woman presents to a gynecology clinic with progressively heavier menstrual bleeding and pelvic pressure for 1 year. On the first 2 days of menses, she changes pads every 1–2 hours. She reports occasional dyspareunia and denies intermenstrual bleeding, postcoital bleeding, or weight loss. Pelvic examination shows a firm, nontender, irregularly enlarged uterus with a bosselated contour. Transvaginal ultrasound shows a well-circumscribed, solid hypoechoic mass arising from the myometrium that distorts the uterine contour. What is the most likely diagnosis?
