A 45-year-old woman (G5P5) reports 12 months of progressively worsening secondary dysmenorrhea and heavy menstrual bleeding. Her periods last 8 days with clots. Pelvic pain is worst during menstruation. She has completed childbearing and denies intermenstrual or postcoital bleeding. Pelvic examination shows a diffusely enlarged, symmetrically tender uterus with a soft, boggy consistency and no discrete masses; adnexa are normal. Endometrial biopsy shows proliferative endometrium without hyperplasia or malignancy. What is the most likely diagnosis?
