She had a deep burn at the same site 25 years ago that healed with a stable scar until recent changes. Examination shows a 4 × 5 cm irregular, indurated ulcer with raised everted edges and a granulating base over a depigmented scar; regional lymph nodes are mildly enlarged and firm.
