A 45-year-old man is brought to the emergency department after a high-speed motor vehicle collision. He has severe pelvic pain and deformity. A pelvic binder is applied. Focused abdominal ultrasound shows no free fluid. Despite 2 L warmed IV crystalloid, he remains hypotensive. CT abdomen/pelvis with contrast (already performed) shows unstable pelvic fractures with a large retroperitoneal pelvic hematoma and no intraperitoneal solid-organ injury. He remains hemodynamically unstable. What is the next most appropriate step in management?
