A 38-year-old multiparous woman at 39 weeks’ gestation with an antenatally noted low-lying placenta has a spontaneous vaginal delivery of a healthy newborn. The placenta is delivered completely and appears intact. One hour later, she develops sudden heavy vaginal bleeding with passage of large clots. Speculum examination shows no obvious cervical or vaginal lacerations. Bimanual examination shows an enlarged, soft, boggy uterus palpable above the umbilicus. What is the most likely diagnosis causing her postpartum hemorrhage?
