She has not had antenatal ultrasound assessing placental location. On examination, the uterus is relaxed and non-tender. Cardiotocography shows a reassuring fetal heart tracing. Speculum examination confirms ongoing vaginal bleeding; cervical assessment by digital examination is deferred until imaging. What is the most likely diagnosis?
