A 55-year-old man with a 15-year history of poorly controlled type 2 diabetes has 3 months of early satiety, postprandial nausea, and intermittent non-bilious vomiting of undigested food, with 4-kg unintentional weight loss. Symptoms are worse after meals and he has no hematemesis or melena. Physical examination is unremarkable.
