A 19-year-old nulliparous woman has 2 years of progressively worsening crampy pelvic pain only during menses. Cycles are regular. Pain begins a few hours before bleeding, radiates to the lower back and thighs, is worst on day 1, improves by day 3, and resolves after menses. She has no intermenstrual pain, dyspareunia, or abnormal bleeding. Pelvic examination is normal. What is the most likely diagnosis?
