A 29-year-old G2P2 woman has a spontaneous vaginal delivery at term. Ten minutes after placental delivery, she develops heavy vaginal bleeding. On bimanual examination, the uterus is boggy. Uterine massage and high-dose IV oxytocin do not control the bleeding. She is receiving rapid blood transfusion but continues to bleed and remains hypotensive.
