A 27-year-old G2P0 woman at 7 weeks of amenorrhea presents with 6 hours of progressively worsening left lower abdominal pain and scant brown vaginal discharge. She has not had an ultrasound in this pregnancy. She denies passage of tissue or clots and reports mild dizziness without syncope. Pelvic examination shows a closed cervical os with minimal brown discharge, mild left adnexal tenderness, and a uterus not significantly enlarged. What is the most likely provisional diagnosis?
