A 27-year-old G2P0 woman at 7 weeks’ gestation by LMP presents with sudden severe right lower abdominal pain, dizziness, and light vaginal spotting. She is pale and diaphoretic with right lower quadrant rebound tenderness and guarding. Transvaginal ultrasound shows no intrauterine gestational sac and free pelvic fluid.
