A 44-year-old G3P3 presents to the emergency department with sudden heavy vaginal bleeding for 6 hours, soaking pads every 20–30 minutes and passing large clots. She reports progressively heavier menses for several months. She has no history of bleeding disorders and takes no medications. Speculum examination shows brisk bleeding from the cervical os without vaginal or cervical trauma. Bimanual examination reveals an enlarged, mobile uterus. Two large-bore IV cannulas are placed and IV crystalloids are started. What is the most appropriate immediate management?
