A 62-year-old man with long-standing type 2 diabetes presents with a plantar ulcer at the tip of the right great toe for 3 months. He denies fever, chills, and rest pain. Examination shows a thick callus with a small superficial central ulcer, no purulence or malodor, and no exposed tendon or bone. Dorsalis pedis and posterior tibial pulses are palpable, protective sensation is reduced, and there is no surrounding cellulitis. Plain foot radiograph shows no bony destruction.
