An 80-year-old woman has had a right groin swelling for 6 months, gradually enlarging and causing discomfort with walking and coughing. It does not fully reduce when supine. Examination shows a 3-cm, non-tender, firm swelling just below and lateral to the pubic tubercle. The mass is irreducible with minimal cough impulse and no skin changes. Findings suggest a femoral hernia. What is the most appropriate management?
