A 32-year-old man with Crohn’s disease had elective ileocecal resection removing 70 cm of terminal ileum 4 weeks ago. He now has watery, non-bloody diarrhea about 5 times daily with mild urgency, without fever or abdominal pain. CT enterography shows a patent anastomosis and no active inflammation. Stool culture, C. difficile PCR, and ova/parasites are negative. What is the most appropriate treatment for his current diarrhea?
