A 28-year-old primigravida at 24 weeks’ gestation reports 1 week of increasing exertional dyspnea. She has no chronic illnesses and takes prenatal vitamins only. Ultrasound shows a monochorionic diamniotic twin pregnancy: one fetus has oligohydramnios with a nonvisualized bladder and growth restriction, and the co-twin has polyhydramnios with an enlarged bladder.
