A 30-year-old woman presents 3 days after vaginal delivery with progressive dyspnea at rest, orthopnea, paroxysmal nocturnal dyspnea, and increasing bilateral leg swelling. Examination shows respiratory distress, displaced apical impulse, S3 gallop, bibasilar crackles to mid-lung fields, and pitting edema to the knees. ECG shows sinus tachycardia.
