A 6-month-old girl is brought to clinic for poor feeding and intermittent episodes of “turning blue.” Her parents report worsening cyanosis during feeding or crying that improves when she is calm. Pregnancy and delivery were uncomplicated. She is small for age and mildly centrally cyanotic at rest. Cardiac examination reveals a harsh systolic ejection murmur best heard at the left upper sternal border and a loud, single second heart sound. There is no hepatomegaly or peripheral edema.
