A 14-year-old girl has irregular, painless vaginal bleeding every 3–8 weeks for 6 months. Menarche was at age 13. Bleeding is not heavy and there are no clots. She denies dysmenorrhea, pelvic pain, fever, weight loss, easy bruising, or epistaxis. She takes no medications and has no chronic illness. She appears well, with age-appropriate Tanner stage.
