A 39-year-old G8P5 at 38 weeks has induction of labour and delivers a 3.4-kg male infant via vacuum-assisted vaginal delivery after a prolonged second stage. Soon after placental delivery, she develops brisk vaginal bleeding. The uterus is firm and well contracted. The placenta is delivered intact with complete membranes. Speculum and bimanual examination show an actively bleeding linear tear along the lateral vaginal mucosa, and the cervix appears intact. What is the most likely cause of this postpartum haemorrhage?
