A 45-year-old woman presents with 1 year of gradually worsening pain, swelling, and early-morning stiffness in both hands and wrists. Stiffness lasts about 90 minutes and improves with activity. Examination shows symmetric swelling and tenderness of the metacarpophalangeal and proximal interphalangeal joints with sparing of the distal interphalangeal joints.
