A 50-year-old perimenopausal woman has 1 year of heavy, painful menstrual bleeding in every cycle. She had an open myomectomy 12 years ago. She denies intermenstrual or postcoital bleeding and has no unintentional weight loss. Bimanual examination shows a uniformly enlarged, mildly tender uterus with no adnexal masses. Transvaginal ultrasound shows a diffusely enlarged uterus without discrete masses and a normal-appearing endometrium. What is the most likely diagnosis?
