A 32-year-old woman has 2 years of primary infertility despite regular unprotected intercourse. Menses are regular but have become progressively heavier, with dysmenorrhea. Pelvic examination shows a mildly enlarged, irregular uterus. Transvaginal ultrasound and hysteroscopy show multiple intramural and submucosal uterine fibroids distorting the endometrial cavity. No tubal factor is identified, and her partner’s semen analysis is normal. She desires future fertility. What is the most appropriate management to help her get pregnant?
