A 50-year-old multiparous woman has progressively heavy, regular menstrual bleeding for 12 months with large clots and fatigue. She has completed childbearing and does not desire future fertility. A 6-month trial of combined oral contraceptives and tranexamic acid did not control symptoms. Pelvic examination shows a mildly enlarged, non-tender uterus. Transvaginal ultrasound shows a 7 × 7 cm intramural uterine mass distorting the endometrial cavity, with no features suspicious for malignancy. What is the most appropriate management?
