A 12-year-old boy has 2 days of gradually worsening right scrotal pain and swelling. He denies fever, dysuria, and abdominal pain. Examination shows an erythematous, edematous right hemiscrotum. The right testis has a normal vertical lie and is not high-riding. Maximal tenderness is at the superior pole, where a small firm tender nodule is palpated.
