A 26-year-old G2P0 woman at approximately 7 weeks gestation presents with 6 hours of light vaginal bleeding and intermittent crampy left lower quadrant pain. Pelvic examination shows a closed cervical os, scant blood in the vault, and mild left adnexal tenderness without a palpable mass. Transvaginal ultrasound shows no intrauterine gestational sac and no obvious extrauterine gestation.
