A 40-year-old G9P6 woman, 9 weeks pregnant by last menstrual period, presents with 2 days of light vaginal spotting and mild lower abdominal cramps. She has no passage of tissue, dizziness, or syncope. Speculum examination shows scant blood; the cervix is closed. The uterus is mildly enlarged and nontender. Transvaginal ultrasound shows a single intrauterine gestational sac with an embryo measuring 5 weeks by crown–rump length and no fetal cardiac activity detected. She is Rh positive.
