A 24-year-old nulliparous woman has progressively worsening dysmenorrhea and deep dyspareunia for 2 years. Her cycles are regular, but pelvic pain starts 1–2 days before bleeding and continues through menses. Scheduled NSAIDs give minimal relief. Laparoscopy shows multiple superficial peritoneal implants and adhesions involving the uterosacral ligaments and ovaries. She wishes to preserve fertility.
