A 10-year-old boy with systemic lupus erythematosus has 3 months of worsening pain and swelling in both hands and wrists with >1 hour of morning stiffness and difficulty writing. Examination shows swollen, tender metacarpophalangeal and proximal interphalangeal joints bilaterally with decreased grip strength and mild ulnar deviation. With cold exposure, his fingers turn white then blue, followed by redness and tingling on rewarming. An overlap connective tissue disease is suspected.
