A 35-year-old man with a prior open appendectomy presents with 3 days of colicky abdominal pain, progressive distension, bilious vomiting, and absolute constipation. He appears dehydrated. The abdomen is markedly distended with high-pitched bowel sounds and right iliac fossa tenderness without guarding. Erect abdominal radiograph shows multiple air–fluid levels with dilated small-bowel loops and a collapsed colon. He is planned for emergency exploratory laparotomy.
