A 30-year-old nulliparous woman has 1 month of progressively worsening dull, constant right lower quadrant and low back pain that worsens after intercourse. She has irregular menses with heavy bleeding lasting 7–8 days. Pelvic examination reveals a tender, well-circumscribed right adnexal mass. Transvaginal ultrasonography shows a 9 × 9 cm heterogeneous right adnexal mass with echogenic areas and calcifications, suggestive of a dermoid cyst, without torsion or rupture.
