A 1-day-old newborn is delivered at 41 weeks’ gestation to a mother with poorly controlled diabetes. Shortly after birth, the infant develops severe respiratory distress with intermittent apnea and poor feeding. Point-of-care glucose is 28 mg/dL (normal 50–126). The infant appears lethargic.

- BP: 62/38 mmHg
- HR: 168 /min
- RR: 68 /min
- SpO2: 86% (RA)
