A 19-year-old man is brought to the emergency department after being struck in the temple during a soccer match with brief loss of consciousness. He regains full orientation, but 1 hour later develops sudden severe headache, repeated vomiting, progressive confusion, and decreased arousal. Examination shows a dilated, nonreactive right pupil and left-sided weakness. Noncontrast head CT shows a biconvex (lens-shaped) hyperdense collection in the right temporal region that does not cross suture lines with midline shift.
